Diabetes Prevention Program
Join the Healthe Clinic for 12 months of prevention
Associates and their dependents (over 18 years) have the opportunity to join the Diabetes Prevention Program through the Healthe Clinic. With the first session beginning in September 2022, this fully virtual program is a one-year commitment to health, with members participating in a total of 22 online group sessions - all at no cost. Healthe Clinic coaches will help guide you each step of the way throughout this journey. the Healthe Clinic for 12 months of prevention
The program curriculum is approved by Centers for Disease Control and Prevention Diabetes Prevention Recognition Program. Sessions will be recorded; however, it is advised to not miss a session. Emergencies will be an exception. All sessions must be completed to receive certificate of completion and qualify for incentives. This program is not eligible for Healthe Living with Rewards points.
Who should participate
- If you are pre-diabetic or diabetic and or anyone with a chronic disease – overweight, obesity, high blood pressure, high cholesterol.
- Open to any US associate (and dependents 18+) looking to make a change in their health and want accountability and guidance (open to all, regardless of enrollment in a Cerner Health Plan)
- Take the Prediabetes Risk Test as provided by the American Diabetes Association and the Centers for Disease Control and Prevention.
